People with Advanced Diseases
Introduction
Having someone to help people with advanced diseases and their families deal with their difficult situations is a great support. To complement attention from professionals in palliative care, "la Caixa" Foundation has created the programme of Integrated Care for People with Advanced Diseases, which has trained 29 multidisciplinary teams that comprise psychologists, social workers and nurses who work to improve the quality of life of both patients and their families. The programme now has teams in 61 health centres throughout Spain who have attended over 15,300 patients and over 24,800 family members.
